blogIf you’ve applied for a job at McDonald’s recently, odds are you didn’t speak to a human being. You spoke to Olivia, an AI-powered chatbot built by Paradox.ai. Olivia screens candidates, collects personal information, and routes applicants through assessments that help determine their fitness for future employment.
Another thing applicants don’t know: virtually every conversation they have with Olivia, along with their personal data, was exposed due to simple security flaws.
Researchers recently revealed they were able to access up to 64 million records through vulnerabilities as basic as guessing an admin username with a password of “123456.” Researchers also noticed that McDonald’s blamed its third-party provider. As a result, the provider published a blog acknowledging the breach, the public shrugged and moved on.
But we shouldn’t just move on.
This breach wasn’t just related to an insecure chatbot. It was a masterclass in what's fundamentally wrong with the way enterprises adopt AI and manage third-party risk.
Dystopia is Already Here
Imagine your teenage child applies for a minimum-wage job. They’re screened by an AI that misunderstands them, misjudges them, and (thanks to a weak password) ultimately exposes their personal data to the internet.
If that sounds like dystopian satire to you, I agree. Unfortunately, it’s real and not an outlier.
We’re rushing to automate some of the most human processes in our organizations—from hiring and health triage to customer service—without establishing guardrails, transparency, or even basic oversight. The result is a widening gap between digital trust and operational speed, with our sensitive data paying the price, post-breach.
Trust-by-Default Is the Real Vulnerability
The McDonald’s incident is yet another reminder that most organizations don’t have a good handle on their trust fabric. AI is rapidly becoming the interface between our companies and the outside world, yet the systems powering it are rarely secure-by-design. And AI vendors are rarely held to enterprise-grade security standards.
Let’s call it what it is: trust-by-default.
Paradox.ai’s AI platform was entrusted with tens of millions of records containing personally identifiable information (PII). Yet its authentication mechanisms were so trivial they would fail a first-year cyber hygiene audit. However, this isn’t a vendor problem, It’s a strategic governance one. We trust AI because it’s convenient. We trust partners because they pass a security checklist (and often only annually, or worse, once!). Ultimately, we trust platforms we don’t understand because they sit behind a glossy UI.
That’s not trust. It’s negligence.
AI, Identity, and the Expanding Blast Radius
As AI becomes deeply embedded in customer, employee, and partner interactions, we’re seeing the convergence of three risk vectors:
Third-party software supply chains, where breach pathways are often invisible to the enterprise
AI-driven automation, where human oversight is minimal and opaque
Digital identity exposure, where a compromised account (human or machine) is often all it takes to gain access to everything
We’re no longer talking about ransomware alone; we now have to consider unverified models making critical decisions and identity systems that assume good intent. If a bad actor discovers a weak admin password or a blind spot in your vendor stack, suddenly your most human processes become your biggest liabilities.
Build for Trust Failure, Not Perfection
The security model of the future doesn’t start with firewalls or detection. It starts with the assumption that trust will be broken by any number of actors—a partner, a model, a misconfigured AI system.
What matters is what happens next.
Can you contain the blast radius?
Can you pinpoint what was accessed and by whom?
Can you recover user and system trust without resorting to guesswork?
This is what trust fabric resilience looks like. It’s not sexy, but it’s the difference between maintaining your high-ground and making headlines.
The Missing Link: AI Without Data Governance Is Just Accelerated Risk
We talk a lot about AI model safety, bias, and explainability. But we talk far less about the data pipelines that feed these models in the first place.
That’s the real blind spot. Today, most enterprises cannot answer basic, critical questions about their AI workflows:
Where did this training data come from?
What sensitive information is being ingested, and by which model?
Can we prove that this AI hasn’t seen regulated, proprietary, or toxic data?
Without visibility, classification, and control over your data—from training to inference—you’re not governing AI. You’re simply hoping it behaves itself.
This is why AI data governance must become a board-level issue, a factor critical for compliance. It’s about survivability; A single model trained on improperly governed data can cause reputational, regulatory, and financial damage that rivals the worst ransomware events.
The enterprises that will thrive in the AI era are the ones that treat data security as the foundation of AI trust. They’re mapping data lineage across workloads, enforcing access policies on sensitive content, and ensuring they can recover and quarantine AI-generated output that poses downstream risk.
We need to stop thinking about AI as “just another application” and start treating it as a dynamic consumer and producer of sensitive data. That means architecting AI-specific guardrails, immutable audit trails, and recovery capabilities that are built for AI-native environments.
That’s where the future is headed and for organizations who understand this now, it’s a competitive advantage, not a compliance scramble.
A Thought To-Go
The McDonald’s breach won’t be the last of its kind. In fact, it’s likely one of many we haven’t heard about yet.
But it’s a wake-up call; not to slow down AI adoption, but to accelerate resilience thinking. Automation is inevitable. Breaches are predictable. But what defines leadership now is whether we’re building systems designed to fail well, are recoverable, traceable, and above all, trustworthy.
To get in front of this inevitability, here are four questions every board of directors and CxO team should be asking:
What AI platforms (internally or externally) have access to PII, IP, or decision-making authority?
Do we know how those platforms are authenticated, monitored, and governed?
What’s our visibility into third- and fourth-party access paths across the digital supply chain?
If one of our partner systems is breached, can we recover data integrity and restore trust with certainty?
Take the time to answer these questions. Ultimately, when trust breaks, it’s not the AI that gets blamed. It’s you.
